-A 34 year old male , chronic smoker, with h/o loss of speech and decreased movements in the upper limb of 15 min duration.
-no h/o of loss of consciousness or seizures.
-pt noticed this on doing work.
-no improvement in the last 15 min.
past history:diagnosed to have a cardiac disease and was on rheumatic fever prophylaxis...
have a discussion .wanted a complete differential diagnosis management regarding this case...

Joseph, it might help to get some x-rays of your cervical and upper thoracic vertebrae. Your symptoms sound like possible transitory blockages in your nerve transmissions at C-5, C-6, C-7 and T-1. If so, a good chiropractor should be able to help you find relief. You're a bit young to be experiencing degenerative changes in your spine, but certainly it's possible, depending on what kind of work you're doing that triggers the symptoms you describe.

As this dates back to June, and is short on clinical detail it is difficult to be helpful.
It may have been a TIA, or the onset of a more serious condition. By now we hope a diagnosis and treatment is under way.
