TRIP new medical database
I usually just use pubmed, sometime I have to resort to Google Scholar, but this database is more medically orientated so you can make the searches more specific to your particular problem.
There doesn't seem to be any restriction on who can use it. So I've signed on and had a look round. It's very fast and seems to bring up quite interesting stuff.
