I've thought about writing a book multiple times and I probably will write one eventually. The only reason I haven't yet is because I have trouble getting everything in my head down on paper in an eloquent manner. I wouldn't even know where to start if I tried to write a comprehensive book on health.
When I do write a book, though, I will give it away for free. I'm strongly opposed to people having to pay for information on how to get healthy - which is why I prefer to give links to free ebooks whenever possible rather then recommending a book you'd have to buy.
The 4 causes I listed are the big ones; there are a few extraneous factors with small effects that add up, like the high heels. High heels could never cause cancer alone, but in conjunction with all of the other burdens we place on our body they do harm your health and thus slightly increase your chance of getting every disease, not just cancer. They also inhibit your ability to recover from diseases for the same reason.

There are a few errors in the last link: Negative ions do not weigh much less then positive ions, alkalized water isn't a good idea, and the chart listing the average amounts of negative + positive ions in certain areas has the 2 values mixed up. And whereas showers do produce a lot of negative ions, if you shower with municipal/tap water the damage to your health due to chlorine, fluoride and other toxins far outweighs any benefits you might accrue. In on warm shower you absorb more toxins then you would by drinking 8 glasses of tap water, some people have estimated you absorb as many toxins as 50 glasses of water
Negative + positive ions are also tied to EMR; most electronic devices produce an abundance of harmful positive ions. Always get outside and stay out there whenever possible, as almost all indoor air quality is horrible. Also get an air purifier for indoors if you have the money, they are expensive but definitely worth it.
Jobs where you work in a cubicle next to dozens of other people, each with a computer, expose you to huge amounts of damaging EMR and huge amounts of positive ions.
The best air purifiers are photo catalysis units (very good but very expensive), negative ion generators, and ozone generators. They're all very good and all have advantages and disadvantages.
