This episode is amazing. Lew interviews a surgeon who, with several other surgeons, were libertarian minded and started their own practice in US, they would not accept any government money (which they thought of as theft) and knew would drive their cost and prices for surgeries way up. Anyway, they are super successful and people from canada are coming down to their little surgery center to pay out of pocket (cost is way less than hospitals charge) for surgery instead of waiting a year or two in canada.

They actually list their prices for surgeries. Since they don't have any government interference to distort prices, they can determine what the actual market costs are of surgeries. Also, their employers are the actual patients, not third parties like HMOs or medicaid.
