Health Insurance companies appear to be restricting the network areas of their policies in some plans under the new Affordable Care Act.
In the past most had a network covering a whole state, but now in many plans they are restricted to designated smaller areas . This means that any treatment outside the new network will become liable to be paid by the themselves.
People should now be particularly careful, and make a point of finding out from insurers the extent of their network, as well identifying hospitals they can access under any plan taken out.

It seems that you must also make sure that not only the hospital but all the doctors who treat you are 'in the network' or you could end up with a big bill.
