I would more than double that to a total of 2G EPA + DHA. I'd suggest something like
3 of these daily or
couple of small teaspoons of thisbut as omega 6 actively displaces omega 3 you also have to seriously reduce the amount of omega 6 oils you are consuming. It really means eliminating industrially made Corn, soy, sunflower safflower oils and everything that contains them. Currently omega 6<>omega 3 ratios in the US are up to 20<>1 the safest level is below 4.5<>1 even in the UK current status is 12<>1 so we need to do just the same.

and a 5000 IU Vit d (one daily for a month followed by once a week after that).

I doubt that will be sufficient. If you look at this article
Startling Findings About Vitamin D Levels in Life Extension Members By William Faloon you see what happened in real life when people only took
5000iu/daily /D3 for 3~9 months. Levels were only just above 42ng/ml.
1000iu/daily for each 25lbs you weigh is generally adequate but checking with a 25(OH)D test after 3months will confirm if this is adequate for your body.
Remember to take oil based gel caps and WITH the largest meal of the day for optimum uptake. Correcting
magnesium deficiency as you correct vitamin D deficiency is a good idea as the extra magnesium will counterbalance the extra calcium your body will be able to absorb as your vitamin D status rises.
For the suggestions I've made Vitacost or Amazon are possibly cheaper for US readers but I use IHERB as their shipping to the UK is cheapest. Code WAB666 saves $5 don't order 18 or more as you get taxed.
